The Client & Home Assessment Visit

Stay at Home is dedicated to supporting seniors and individuals with physical challenges through our unique, customized combination of services, enabling our clients to continue living at home – with confidence.

Making sure that just the right services are delivered begins with the Client and Home Assessment visit. This initial meeting combines the expertise of a trained Personal Advocate, who has a minimum of a bachelor’s degree in social sciences, and an experienced Home Specialist, along with the client, and/or the client’s family and friends.

The Personal Advocate meets with the client and/or the client’s family members initially to develop a customized support plan to best address the client’s specific needs – with a focus on the quality of their lifestyle.

During this initial visit, the Personal Advocate evaluates all facets of the client’s life at home and matches that evaluation with support services needed in the areas of lifestyle, home and financial.

The Home Specialist conducts a thorough assessment of the client’s property, including house and yard, and recommends changes that will make the home safer and more comfortable. This home assessment covers everything from doorways, stairs, bathrooms and kitchens, to electrical wiring and plumbing, with the focus on identifying problems before they become an emergency.

The Client and Home Assessment visit also helps establish rapport and confidence between Stay at Home and the client. Our goal is that we all come out of the initial meeting with a strong feeling of trust and a comfortable working relationship in place.

After the initial meeting, the Personal Advocate produces a comprehensive report detailing the client’s needs and recommended services, as well as our proposed list of service providers. If requested, a follow-up meeting can be held to review the report.

Stay at Home understands that a client’s needs will change over time. The Personal Advocate’s service plan can be reviewed and revised at any time, upon request.
